3.         Resume

4.         Photocopy of Transcript of Records (Authenticated)

5.         Photocopy of Certificate of Trainings and Seminars attended

6.         Authenticated Photocopy of Board Rating/CS Eligibility (if any)

7.         Photocopy of Certificate of Previous employment (if any)

8.         Copy of duly signed IPCR/Summary of Ratings (for insider only)
